ADHD Medication & Titration Tracker

Every appointment asks how it has been, and you cannot honestly remember past the last few days.
Titration asks you to notice small changes across weeks and then describe them accurately to somebody in ten minutes, weeks after they happened. That is a lot to ask of anyone's memory. This 10-page printable tracker does not try to work out whether a dose is right — that belongs to your prescriber, with your history in front of them. It is here so that when they ask, the days are written down.

Who this is for
This tracker is for anyone living with adult ADHD who wants a record of what actually happened, rather than trying to recall it later. Every appointment asks how it has been, and you cannot honestly remember past the last few days. If that is familiar, this is the tool for writing it down as it happens.

Inside the planner

How it works in practice
The unit here is the DOSE PERIOD, not the week or the month — a new block starts when the dose changes. Each day records the dose, the time taken, focus, whether you ate normally, how you slept and how the late afternoon went, because those are the ones people cannot recall at an appointment and the ones that get asked about. The side-by-side page then puts the dose periods next to each other, so ten minutes with a prescriber starts from a record rather than an impression. The baseline page is written before anything changes, on purpose: almost everyone underestimates how they were beforehand once something has shifted.
